When considering breast augmentation, you may be concerned about the appearance of scars from the procedure. This is an important consideration, but less so than you may think. Breast Augmentation Incision Locations
Breast augmentation incisions are used to insert the breast implant. They are relatively small, generally only about an inch in length, and are commonly placed in one of three locations:
- At the crease below the breast (inframammary)
- Under the nipple-areola complex (periareolar)
- In the armpit (transaxillary)
There is also a fourth incision option, what is called Trans-Umbilical Breast Augmentation (TUBA), where breast implants are inserted via the navel (belly button), but the increased risk of complications associated with the procedure means this option is not commonly used.
Advantages of the Inframammary Incision
The inframammary incision is the most commonly used incision because of its numerous advantages. This incision gives good visibility of the implant pocket for the cosmetic surgeon, which contributes to generally better results. For most women, the incision is also very well concealed, since it will be under the breast. Distance from the nipple/areola complex may mean a lower risk of nipple-related complications and potentially a lower risk of bacterial contamination that leads to capsular contracture. Inframammary incisions may not be a good option for women with poorly defined breast creases.
Advantages of the Periareolar Incision
The periareolar incision is made just under the nipple-areola complex, and is concealed in the change of height, color, and texture between the areola and breast. It also allows good visibility of the breast pocket and encourages good surgical results. Some studies suggest this incision may be associated with a higher risk of breastfeeding difficulties, changes in nipple sensitivity, or capsular contracture, but other studies say not. It is likely increased complication risks are associated with an individual surgeon's technique rather than choice of incisions.
Advantages of Transaxillary Incision
The transaxillary incision is the only option for an incision that is not on the breast itself. Because the breast implant has to be moved through the incision under the arm to the breast, it can only be used with saline breast implants or smaller silicone gel breast implants. It can be used for either subglandular or submuscular breast implant placement. If you are considering this incision location, make sure your surgeon is experienced utilizing it, because poor surgical technique can lead to asymmetry. Finally, remember that this is the only incision location you are likely to expose in public, so if you do get unfavorable scarring, your clothing options may be limited.
