We make every effort to minimize the cost of your medical care. To reduce the cost of billing, we request payment in full at the time of your visit. All elective cosmetic procedures must be paid in advance. Our practice accepts VISA, MasterCard and American Express. Please be certain to provide the office with all of the required referral and authorization forms prior to, or at the time of, your visit. Without the necessary referral forms, payment for services rendered is due at the time of your appointment. We do have a 24 hour cancellation policy.
